    Sorry for the very late answer.
    This issue happens because both URLs that you shared are on the homepage. That is the homepage with just one or more query arguments added to the URL.

    I suspect you disabled Fluent Forms on the homepage. If Fluent Forms is disabled on the homepage, if you go to those URLs, you will see nothing provided by Fluent Form.

    For solving this issue you should:

    • Go To Freesoul Deactivate Plugins => Custom URLs => Frontend URLs
    • Add a row with the pattern *?fluent-form=*
    • Keep active Fluent Form on that row (disable all the other plugins you don’t need)
    • Add a new row with the pattern *fluent_forms_pages=*
    • Keep active Fluent Form also on that row
