Confused by Jituzu Tool

I’m trying to configure Jituzu tools.

What do I add for provider?

(There really should be a hint in parentheses.)

I tried first name and last name with no space, but this isn’t correct.

Somewhere I read enter practice that is listed in Settings from jituzu.com, but what I see is last name first name with a comma in between and this doesn’t seem right…
