Confused on skinning WP (where to start)

I notice all the themes are centered, is it possible to make one that is left aligned? And as far as skinning WP, do I build my CSS page first like in DW and get it working or do I build it piece by piece from my image? I have been reading through the documentation and I am just confused on how start making my own theme that is left aligned. Any insight is greatly appreciated.
