Contact form 7 feezing on submit but still sends message but not to mp

Hi
I am using your mailchimp plugin on my wordpress site and linking it to my contact form 7 form with the id and APi.
We are having problems when we click submit, the form freezes shows an action animation but goes no further, we recieve an email from contact form7 but it seems nothing goes to mailchimp.
We have two forms on the site, they use the same APi and Id and the same fields, could this be a conflict ?
Do we need a mailchimp database for each form?
I used the same fields in each form hoping it would wright entries to the same mailchimp database.
All The fields have mc4wp- as a prefix i.e [mc4wp-email]
It mentions having a checkbox.
Is this esential ?
