contact form 7 issue, message sent ok but not received email yet!

so I have created a course evaluation form using contact form 7 and this is how it looks https://www.cadmicro.com/solidworks-course-evaluation/.

All my other forms on the website work properly except this form. ive recieved 2 submissions out of the 10 I have submitted.

the message states its been sent but I dont get the email.

ive tried deactivating plugins such as mymail, quick cache, redirect etc. and nothing is making a difference.

the times I have receieved the form submissions all my plugins were functional and activated.

I am stuck and not sure what to do to fix this issue and make sure all submissions go through properly.

i placed wp-config file on debug mode and this what i keep gettign everytime i try to save my form after editing.
