Contact Form 7 with nouislider for Range Slider

Hi All,

I need to implement a range slider in to a contact form.
I have found this one here which is free but I don’t know how to implement it to my site.

Does anyone know how to do this?
Or, does any one know of a form system for WordPress which does this already?
