contact form and phpmyadmin database

There are many more plugins . Just search for contact form in the plugin directory.

Actually I tried this plugin and also I tried Formidable plugin, and data is stored in database, but the problem is that I want that data to be set clearly by order for e.g.(name, email, phone..) and than all that data to be saved in my personal email? Any Idea?

Ask authors of those plugins at the plugins pages – they might be able to customize for you.
