Contact Form in page doesnt work?

Hi!
I want to have a contact form in one of my pages but it doesnt seem to work. Check out: https://www.poisonpeas.com/index.php?page_id=6

As you can see it’s not really worked out properly. Here’s the original code:

It might not like it because i have extra head tags in there, but i need those for the javascript?
Any idea’s?
