Contact Form Not Working – Appears Scrambled

I am new to WordPress, this is my first WordPress site and I am building it for a client, it needs to be finished really soon, so desperate to get this issue sorted out.

I have installed the Form that comes with Jetpack, when I create the form in the Booking form page it appears all over the place:

Any ideas on why this is? How do I resolve it?
