Contact Form to DB plugin issue

I’m using Contact Form DB for the first time and am very confused how to actually create a form — anyone have experience? I need to create what I think is a fairly simple form the client fills out and it exports to a spreadsheet that the admins can view and edit. It’s going to be on https://www.sevenlevelsent.com/ but that’s probably not relevant at this point since nothing is live that incorporates this plugin.
