Contact Form with Conditional Instances

Hi, everyone I want to know if there′s a contact form plugin that has conditional instances, for example:
1. A visitor want′s to contact sales so he select in a dropdown box sales department and then we have 3 salesman and 1 manager of sales so the plugin select and send an email to 1 salesman everytime a visitor select sales with a copy to the manager.

And the other thing is what plugin show a form in the admin area, if I need that every user enters login the have a contact form in the dashboard.
