Contact form with image and title from referral page

  • I would like to have a contact form on my site that includes some information from the specific post related to the inquiry. I am thinking there may be a code snippet that I can put in the template for the contact page.

    On this site with plants, a visitor could click a link on a post about Agave plants and the contact form they reach would make it clear their inquiry is about Agave plants by including the referring post title and featured image.
