Contact form works on page, but gives red error when embedded into PHP

When contact form is added to page as shortcode – it works fine.
https://kv-omanikult.ee/testBut when same form is embedded to page via PHP (under REPORT MAKLER) https://kv-omanikult.ee/properties/odnokomnatnaya-kvartira-v-lasnamyae-ot-hozyaina/
it gives red error.
Any ideas?
