Contact Forms 7 mixed up between side bar and main page

I am using Contact Form 7 and layout of pages has a side bar and main page.
The side bar form and the main page form are different but swap around when you click on the links from live page.
This happens only with one form ‘Become an OPAS Member’. The sign in form on the side bar remains static and does not get mixed up with other forms on the main pages.
Any suggestions?
