Contact Me With Disclaimer checkbox

I have a site that will allow someone to ask a question and in my contact me section, I would like the ability to ask the respondent to acknowledge reading a simple disclaimer…is there a plugin that can do this?

I understand I could add a checkbox within the contact me page, but it would not be contigent upon sending in question.
