Contact Your Representatives

I have searched 125 pages of plug-ins looking for something that I can set up that will allow users to enter their name, email address and their comments and will send it to the email addresses of our local representatives. I would like it to send the email as if it was the person filling out the form if possible. I can’t find anything like this does anyone know of a plug-in that works like this? The closest thing I found was a petition plug-in but I need something that will send out emails.
