Contract 7 Form Not Showing

Hi All,

Really annoying issue, my WordPress contact 7 form works perfectly, but the email address(which is required) doesn’t show when i receive an email?

my mail option is set as: [your-name] <[your-email]>

and my message body is:

From: [your-name] <[your-email]>
Telephone: [your-telephone]
Company: [Company]
Product: [product-name]

Message Body:
[your-message]

Please help!!
