Control over cancel membership

Hi,
As an admin, I want to keep control over cancel the membership. The user should request over email or by filling a form to cancel their membership. How is this possible? (1) How admin can cancel a membership from word press dashboard (it should delete all the user info)?

I deleted the membership-cancel page. On membership account page “Cancel” link is still there, how to remove that link+hyperlink?
What else should I do?
