control panel css/js issue

I started having problems with the UI of the control panel a few weeks ago. The edit post screen has the formatting all messed up and half the links do not work (e.g., Text, Visual, fullscreen, etc.) I tried upgrading from 3.6 to 3.8 to see if it would fix the problem, but the problem remains. Things I have already tried that did not help:
– upgrading to 3.8
– turning off all plugins
– changing themes (including the default wordpress ones)
– using the “Use Google Libraries” plugin
– adding the “concatenate scripts false” directive to wp-config.php
– Using a different computer and/or browser (tried 3 computers, 3 difference browsers and 2 different operating systems)A screenshot of how the “edit post” screen appears is here: https://www.holidaynook.com/wp-wtf.jpg
I have also noticed that when I am viewing a post, the admin toolbar does not appear along the top, but rather is along the bottom left, and it is missing formatting. I am not sure if these 2 issues are related, but it sure seems like a css/javascript/jquery issue.
