Convert PDF Product Catalog to digital survey

  • Hello,

    I am trying to add check boxes to a survey, along with a submit button.

    The purpose is to make it so that customers can look at our product catalog and check the check boxes, then press a submit button.

    Then that information would be emailed to us.

    We are thinking about using mailchimp to do this.

    Anyone know of the best way to do this?
