Create a form with ‘IF’ statements

  • I want to create a form where when the customer answers the 1st question, he should get the second one based on what he answered in the 1st one.
    So, IF the answer to the first question is ‘YES’ then a different question and IF ‘NO’ then a different question.
    Any idea how to achieve this?
