Create a membership registration form

Hello, can I create like this registration form “https://anthemes.com/themes/kickcube/box2/register/” using this membership plugin. I want membership level show on like this, I am already create this 2 level but can’t add payment option like this when user choose paid one and click on register then at fast need to pay and then they can complete registration after complete payment. How I can do this?
