Create Form in a Dashboard Widget

Hi, i have a custom widget in the dashboard admin of wordpress, in this custom widget i put in a form of contact form, but doesnt work, in a page works perfectly, but in the widget dashboard doesnt sent the information, doesnt show any message either, for an error, or success, only charge a few seconds and then refresh the form like reset the form. Thanks a lot for anybody can help me.
