Creating a Form with Images and Text Boxes

    We need to create a form similar to the one shown in the image, where each section consists of an image combined with text in three boxes. I’ve seen another form in Forminator with a similar layout. Is it possible to create something like this? If so, how can we set it up efficiently?

    Our field that allows images are the Checkbox and the Radio, but after checking the image you shared this seems a job for a radio:

    And enabling the images:

    It should look like this:

    For the pagination, you need a Page Break field.

    Yeah I’ve seen the radio buttom with image optin. But I want a larger picture and I want the radio buttoms gone. So I only have 3 pictures. How to do that?

    Sorry for the delay regarding this, it appears we didn’t get any email notification regarding this response.

    I’m afraid, there isn’t any setting in the plugin to change the default behaviour and it’ll require custom CSS to change this behaviour, which I’m afraid will be outside the scope of support we can provide.

    For that, you’ll need to hire a developer to provide the required custom code for you. WordPress provides a jobs directory here
