Creating a paid support forum for a medical consultations website

I’m creating a [ redundant link removed ], where professional doctors answer questions of patients about their problems for some $3-5 each. Here are my issues:

How to make it that the user asks a question in a simple contact form and chooses the doctors’ specialization, presses ‘ask’, pays those 3-5 bucks in a new window and then only a special kind of doctors is able to answer this question or change the doctors’ specialization who is qualified to answer it (not other users, only a particular kind of doctors).

Also, I will embed a video chat with payments per minute. Here are the ways to embed a video conference on a wordpress website, but do you know how to charge per minutes?
