Creating a website with multiple tests on it

I’m looking to create a WordPress site for a new client who wants about 7 tests like this one in the link below where there is a score that’s created.

I’ve had projects in years before that involved mathematical calculations where a formula was added via coding and I was curious if there was a plugin out there that can help with this. Any and all input would be great!
