Credit Card expiration date defaults and payment won't go through

Hi,
People are trying to sign up on my client’s site: https://withmefit.com/welcome-to-the-members-area/?level=2 and the form resets each time and says that all fields need to be entered. The credit card expiration field defaults to January 2016 – it won’t change.
I’ve tried this with test card numbers and sometimes it works, sometimes it doesn’t.TEST1
First try was 2/2017 FAILED; second try was 5/2018 SUCCESSTEST2
First try was 2/2016 FAILED; second try was 2/2016 SUCCESSPlease help!
