CSV File Setup

I want to use a form developed at emailmeform.com to capture birthday’s from our Facebook group and then export as a CSV to input and display on our website. How would I setup the form to export the CSV correctly, like you suggest – CSV must have format <name>,<date> where <name> a string and <date> as Y-m-D?
The plugin works great for what we need, but just need to capture the birthday’s and input them correctly from the start. Thanks!
