    I have followed the instructions for the custom form, and I made it.

    I made a folder in my theme/usp/ and placed the php form file and the css but when I made changes there, they didn’t apply. So I was forced to make changes in the original files. My question is, when the plugin is updated, will the changes be gone?

    If you made changes to the plugin file(s), yes they will be overwritten next plugin update.

    So how can I make changes in the submission-form.php and apply the changes without losing the changes?

    I’m not sure, some troubleshooting would be useful in determining the cause of the issue. The steps in the plugin docs are known to work well on default WordPress site. That would be the first thing to try: set up a new/clean WP and install/activate only USP. Use the default WP theme. Then try again with the steps under “Custom Submission Form” in the plugin Installation docs. Also don’t forget to set the “Custom Form” option in the plugin settings.
