Custom Contact Form

  • Hello, I am not sure if this is the correct section or not…

    I have a client who runs a dental lab, they want to make their RX form digital. I have tried the fillable PDF online but the submit button does not work in all browsers.

    I was thinking of creating a custom form that will return the results as PDF.
    Below are the requirements from the client.
    1. Results need to be submitted in PDF form
    2. Results need to look like current RX form. This includes images as well. You can see an example here: https://infinitydigilab.com/wp-content/uploads/2020/09/LSF_082020_ID_1pge_INT.pdf
    3. The form would need to have a signature form for the Dr. to use the mouse for the signature or be able to upload a .png or .jpg for the signature.

    Is something like Contact Form 7 with a signature addon able to accomplish this? Or is there a better solution/plugin or service that can accomplish this?
