Custom Contact Form (CCF) – Version 4.7.0.4

Hi, I have 2 custom contact forms on my website and would like to add a third but every time I try to create a new field it doesn’t work. Can anyone offer a solution? What would happen if I deactivated and reactivated the CCF plugin – would I lose my existing forms?
