First, to display a title in the columns, you simply should insert an “Instruct. Text” at beginning of each column using a pair of <h2>Title here</h2> tags as the fields’ labels.

To assign a different background color to each column, enter the following style definition into the “Customize Form Design” attribute in the “Form Settings” tab (https://cff.dwbooster.com/images/documentation/form-settings-tab.png):

#fbuilder .cff-container-field .column2:nth-child(odd){background:red;}
#fbuilder .cff-container-field .column2:nth-child(even){background:blue;}

Of course, replacing the colors code with the preferred ones.
