Custom fill in field — plugin?

  • Hi there, this is my first post to the forum, so I’m not sure if this is the right place for my question, but for a client I am looking to make something like this:

    I want a … party [submit]

    At the dots, i want people to be able to fill in the kind of party they’re looking for. After clicking submit there has to be some kind of small contact form so that my client can contact the person who filled in the form.
    How do I do this?
