Custom Form Fields Disappear in WP

In WordPress > Easy Appointments > Customize…
1) I created 3 custom fields and saved the screen.
2) In preview, I am able to see these fields.
3) In WordPress, once I click off of this Plugin, then click back to Easy Appointments, the custom form fields disappear.
4) I create the custom form fields again. Then preview. And now in preview the form fields show twice.Cannot access/edit custom form fields in WordPress. How can I edit or delete when they are not visible in WordPress?
