Custom Forms

We do custom forms on our wesites, and the standard plugins for creating these are coming up a little short on my wish list.

What i would like to happen is, if the front end radio button shows “mark” as an option, the backend data that it emailed to a recipient is “A” or some special code.

Is there a plugin out there that will handle this ?
