Custom Log In Page Lost

  • I created custom log in pages for my sites using codex instructions from here. Had it running since November/December last year without issue. Had it working well on 4 of my sites.

    Today I noticed it was back to the standard WP admin style on one site. So I checked the code and nothing appeared to have changed. Compared to Codex and it is correct.

    Checked my other sites and 2 more were back to standard.

    Only difference in the four sites is that three of them recently updated to the latest version of WP and the fourth, with the custom login in tact, is one version out of date.

    Has something changed in WP not allowing the creation of a custom log in using functions.php or has the code needed been changed and the codex not updated?
