Custom login box

Hello,
How tocreate custom loginbox?

I would like to use custom login for wordpress users, on my theme.

If you type: host.com/wp-admin or host.com/wp-login what they will be redirect to custom login page.

I need, that users, cand login into site, using my custom theme login box, but how to create this box? I Would like, that they see login box, with username, password, remember me, forgot password buttons.

How to do it with out plugins?
