Custom PDF Using Checkout Manager Form Fields

Hello,

I have form fields in the Checkout portion of my WooCommerce site. it’s above the billing info. I’m looking for a way to create a custom PDF after checkout using the form fields added in the “Additional” tab. Is there a plugin that you (or anyone) is aware of that works well with WooCommerce Checkout Manager?
