Custom Post – Front End

I’m using a custom form on the front end for registered users so they can fill the form with the required upload fields and submit it.

I’ve tried using this form as a page template but it don’t submit the form and even i tried some other ways to include this form by modifying the page.php but still didn’t work.

It however works if you hide the form and use include in header ( But then it will show the form source on every single page ).

Any help on it ?
