Custom registration with terms & condition

Hi,
I want to add extra field in my custom registration from
Name
Licence agreement (I accept / I do not accept ratio button)
Link to Licence Agreementhow to do that…I triede with Pie Register (https://www.ads-software.com/extend/plugins/pie-register/) but it ‘s not working with wp 3.5 version.
Any help will be appreciated…
