Custom Table

Could you make a version that allows to insert the data into custom table inside the WordPress Db.
So, optimally a system something like;
At the bottom of the form design a check box to insert into custom table.
A drop down to select that table.
And then an area to map the form fields to the columns.

Alternatively would you share the source?
