Custom Taxonomy Creation with Gravity Form User Registration addon

I am using Gravity Form + Custom Post + User Registration add-on. I have a form where one of the field is used to create a Custom Taxonomy “Agency Name”. The field type is “Single Line Text field” which is Agency Name, which is used to create Custom Taxonomy Name “Agency Name”.

This form is also used for registration of the agency, using User Registration add-on. The other functionalities of the form is working but the Custom Taxonomy is not getting created. I am not sure why this is happening.

The plugin version used here is >> 3.1.9

Can you suggest the probable reason or how to debug the functionalities.
