Customize Form

  • Hello,
    I am trying to see if I can create a form similar to this:
    https://mindyourmind.ca/submit

    As you can see there are some conditional questions before the form is displayed.
    I have tried using Javascript to create those conditions, but WordPress editor modified the JS every time I edit the page, and therefore the JS stops working.

    Would you be able to suggest a way to do this or tell me if there is any options in the pro version that helps with what I need done?

    We have checked your screen shot. Sorry, you cannot make this type of form that you have provided above screen shot.But we are planning to add this feature in our future updates.But you can make the different form that is available in our template of the anonymous post.
