Customizing survey

I have recently installed this plugin to try out as an alternative to Survey Monkey for my businesses clients and noticed that at present, it only gives me a field for a question and yes and no answer options. Can this be customized to be more of a rating scale of 1 to 5 rather than simply yes and no?
