Customizing the pdb_signup page

I was able to create my custom template in my child theme for the pdb_single page using the example here It lays out just how I wanted it. I would like to do the same thing for the pdb_signup page, however I’m stuck on getting the correct print call for each element.

All I need is the correct print call for one element like first_name, I can figure out the rest.
