Customizing this plugin

This is a great plugin, but im looking to have this plugin customized without loosing the ability to receive updates. this plugin is being used on another website and its absolutely amazing. i would like to recreate the look and feel of it.
here is an example of what im looking to accomplish and what was done, this is the WP Customer Reviews plugin remastered: https://www.njcenteros.com/patient-review-form/
And this is how they are shown after approval:
https://www.njcenteros.com/testimonials/patient-reviews/The form only prints the review rating, first name and last initial, how many stars out of 5 and date. on the backend and notification email will email the entire submitted form.
Just like the example form, i want to replace the rating stars image by using FontAwesome instead.
For the drop down box on the example form, i would like the options to be editable in the administration end and as well as all the yes and no questions.
Any help would be very much appreciated
