I should have mentioned in my original post that I did confirm my permalink settings were configured to the specification in the getting-started document. I just double checked and they are set properly.

Any other idea what might be causing this problem? I’m interested in rolling Dokan into production but can’t unless I can solve this issue.

Without the permalink issue, there should not be other problems coming from Dokan. If you have checked with other themes as well then we need further investigation.
