Data Entry and Retrieval from DB using forms

  • Hi Friends,
    Hope you have seen lot of form entries in many websites.
    I am asked to create a data entry application (and retrieval) for a school. I checked through the plugins and found “formbuilder”, but seems its not what I’m looking form

    The requirements were
    1. Enter the details of a student
    2. When you want to add, you need to select “School name”, this will show “Grade” dropdown which will list all grades. Once “Grade” is selected, the student details can be be entered.
    3. Same way for admins(certain accounts), when you display “Grade” in drop down, it should select all students etc..

    I’m actually looking for something like “https://demo.phpmyadmin.net/STABLE/index.php?db=RW”
    whereby if you select an group, it would show the details of the group
    There is provision for inserting and updating etc..

    Is there a plugin to directly create forms and retrieve from Database?
