Data-spinner html code showing up on form

I have used Caldera forms for a few months now and really like it. Today I have noticed an issue on the site I’m creating. The spinner html code is messed up and shows on the actual contact page.

It appears to be some bad html but I’m at a loss on how to fix it, and it is not happening on any of the other sites I’ve used Caldera with.

I have tried creating a new form with the same results. So I deactivated the plugin and removed it. Then re-installed it and created a new form and it still has this issue.

I am using the newest version of wordpress 4.5.2 (fresh install) with the newest version of the Avada Theme (4.02).

Any Ideas would be appreciated.
