Database

HI guys i have an issue.i created forms in php and then i converted in to the wordpress template,i am changes in databse connection as par wordpress select querys.
On submitting the form i giv action “send.php” values inserting in database but page shows the message “
This is somewhat embarrassing, isn’t it?”
